Welcome to Murrieta Valley High School Girls Basketball Sports Team!

 All New and Returning players must submit the following:

1) A physical signed by a doctor and dated AFTER May 1st, 2022. Get the form from:           ​​          www.athleticclearance.com

2) A current copy of your medical insurance card.

3) Signature page from 
      ​www.athleticclearance.com

Upon completion, please e-mail all THREE items to Ms. Mason (Athletic Secretary) at 
 kmason@murrieta.k12.ca.us
Once received and approved, you will receive an e-mail with an attached clearance card to be turned into Coach Alison, which will then allow you to participate. 

Additional Notes:
1) When creating your account on www.athleticclearance.com, be sure to register for the 2023-2024 school year.

2) You can visit your personal doctor for a physical or stop by I-Care Urgent Care at:
27722 Clinton Keith Road
They offer physicals for $20.00 and are open from 9:00am through 7:00pm
